Patient Summary Care Record
Patient Summary Care Record (SCR)
If you are 16 years or over you can now view your Summary Patient Record online. Your Summary Patient Record comprises of your allergies and sensitivities and your repeat medication.
You need to request that your online Summary Patient Record is enabled before you will be able to see it. You will be required to complete a consent form. Please ask reception for a consent form. You will need to do this in person, and provide reception with an original form of photographic I.D. - your photocard driving licence or passport.
